Rehabilitation Assistant, Haywards Heath
Experience preferable but keen to have the right person for our client
Part-time: 16 hours over two days (one being Wednesday)
We have many hours available at a later stage.
Pay: £12-14 per hour
Role Overview: To provide high quality support to an adult (female) with an acquired brain injury, in their life and rehabilitation programme. This may include helping them to managing their disability, accessing the community, moving home, building relationships and developing their use of technology in order to increase their independence.
Working with this previously active individual there is exciting opportunities to support her maintain her links with the community and maximise her independence through her rehabilitation programme. You will be working with her in her current nursing home placement and so be required to work alongside their staff. There is an expectation that she will be moving into her own accommodation in the community with a full care package at some stage.
What You Will Be Doing:
Working with the rehabilitative professionals involved to provide ongoing rehabilitation under their support and guidance
Working with the nursing home staff to monitor her health and minimise any hospital admissions. If admitted to hospital to act as her advocate.
Assisting the individual with managing and supporting her own independence by encouraging her through the rehabilitation programme appropriately (including supporting participation in the activities designed for her rehabilitation)
Providing person centred services to this individual in all aspects of her life including opportunities to access the community through recreational and social activities and enhancing life experiences
Supporting her to maintain and develop links with family, friends, and other professionals across a wide variety of locations
Assisting the individual to plan her life around her own needs and goals ensuring these are assessed in relation to her lifestyle and taking account of risk management approaches

Knowledge and understanding of 'safeguarding' vulnerable adults
