Gray Healthcare are currently looking to recruit a Community Support Manager to join our national private mental health and recovery provider.The position is based in Lincoln. You will need to be able to drive, have a full clean driving licence and access to a vehicle as you will be required to work with service users across the footprint of your locality.

Job Purpose: You will have experience of leading a team / teams in the delivery of daily support to service users experiencing mental health problems, including those associated with acquired brain injury, personality disorder, and learning disability.

  • To support and direct teams within a defined locality to deliver personalised care.
  • To monitor and assess quality performance measures across the locality footprint.
  • To lead and participate in individual progress of service user care.
  • To present as an outstanding role model, demonstrating continuous personal development whilst keeping up to date with Mental Health nursing and care innovations. Through supervision identify your own development and training needs and locality to deliver personalised care. To monitor and assess quality performance measures across the locality footprint.
  • To lead and participate in individual progress of Service User care.
  • To ensure that staff skills and knowledge of risk assessment and care and recovery plans are effective in meeting the needs of individuals by monitoring performance, competency and individual learning.
  • To ensure that staff develop effective professional and caring relationships with service users in order to promote positive outcomes.
  • To act as a positive role model and coach to the teams.
  • To work with the teams to deliver what is important to the service user and what they want for the future.
  • To work alongside Colleagues, Clinicians, Therapists and all members of the Multi-Disciplinary team to promote safe, effective and responsive care and recovery.Our Views We are committed to delivery excellence in care to all our service users, families, partners in health and social care and our commissioning customers.You will need to be able to work flexibly to meet the needs of the business.Salary & Benefits.
