Assistant Practitioner, Primary Care, HMP Thameside An exciting opportunity has arisen for Band 4 Assistant Practitioners to join the Primary Care team within the healthcare department at HMP/YOI Thameside. Oxleas NHS Foundation Trust provides healthcare services at Thameside, inclusive of primary care services, mental health services and clinical substance misuse services. These roles have come about due to a recent service re-design
HMP/YOI Thameside is a category B local remand prison holding approximately 1200 men aged 18 years upwards. It is a modern prison built only six years ago so and is run by SERCO on behalf of the ministry of justice
The prison boasts a modern gym facility which can be accessed by staff free of charge. There is also a staff canteen. There are free staff parking facilities available for staff and the prison is local to Plumstead and Woolwich Arsenal, so has close access to several bus routes and train stations.
From a professional perspective you will have completed The Health and Social Care Assistant Practitioner Foundation Degree. You will be a highly motivated, dynamic and enthusiastic individual able to deliver exceptional care to people in custody. You will be an excellent communicator and possess good assessment and care planning skills and have a good knowledge of clinical standards.
You will be competent in assessing, planning, implementing and evaluating care and undertaking risk assessments of those in your care
We are looking for staff with a wide range of clinical skills, inclusive of an understanding of wound care, infection control, health screening, health promotion, physical health checks, long term conditions, ECG monitoring, sexual health screening amongst others. There will also within this role, be a focus on providing and managing smoking cessation services, within the prison and taking a lead role within this area
